How this valuation is worked out
Practices run by staff therapists and a clinic manager are priced on EBITDA at roughly 3.5 to 6 times, with multi-clinic groups at the top. Single clinics where the owner still treats are priced on SDE at about 2 to 3.5 times.
A worked example
A single clinic with $1.1M of revenue and $210K of EBITDA after paying a market salary for the owner's treating hours. At 3.5–4.5× that is $735K–$945K; a second location and low therapist turnover would push it higher.

Common questions
Do cash-pay services help?
Yes. Wellness, dry needling and cash programmes carry better margin and are not exposed to payer rate cuts.
Is one clinic harder to sell?
Not harder, but it prices lower per dollar of profit than a group, because a group spreads risk across locations.
Who buys PT practices?
Regional platforms, hospital systems and individual therapists using SBA financing.
