How this valuation is worked out
Licensed, Medicare-certified agencies with management in place trade at roughly 4 to 7 times EBITDA. Private-pay home care agencies, which carry lighter regulation, are usually priced on SDE at about 2.5 to 4 times. Buyers underwrite payer mix, billing compliance and whether the licence transfers cleanly.
A worked example
A Medicare-certified agency with $4M of revenue and $500K of EBITDA. At 4.5–6× that is $2.25M–$3.0M; steady referral sources and a clean billing audit history are what hold the multiple.

Common questions
How long does a sale take?
Licensure and change-of-ownership approval usually set the pace, and that can run several months depending on the state.
Is private-pay worth less?
Per dollar of profit, usually yes — but private-pay agencies are simpler to transfer and attract a wider pool of buyers.
Do buyers check billing?
Always. Documentation and audit history are reviewed closely, and unresolved exposure is usually held back in escrow.
